Academy Nomination Checklist
Request and return precandidate questionnaire.
Register to take the ACT or SAT test.
Photographs attached to application form.
Academy recommendation form, completed by high school.
Official high school transcript for three full years of study.
Scores on either the SAT or ACT.
Essay stating your personal reasons for choosing a service academy.
Send completed application form.
