Becoming an L&I; Provider

Join the Medical Provider Network

You should apply for our network only if you practice in Washington State and are a: Physician, Chiropractor, Naturopathic physician, Podiatric physician, Advanced registered nurse practitioner, Physician assistant, Dentist, or Optometrist.

Apply for a Provider Account Number (Non-network)

If you are not one of the provider types listed above, or you do not practice in Washington State, you should apply for an L&I; account number. Do not submit a network application. You can continue to treat injured workers without joining the network. This includes massage therapists, physical therapists, occupational therapists, speech therapists, psychologists, IME's, vocational providers, and interpreters.

Out of State Provider? Apply for a Provider Account Number Application

If you are a provider in any other state other than Washington, please submit the Provider Account Application so you may be paid for services.

Out of Country Provider? Apply for a Provider Account Number Application

If you are a provider who is Out of the Country, please submit the Provider Account Application (Non-network) for us to pay you for services rendered.

Next Steps

Allow 60 – 90 days for review of your application. We will notify you of our decision. If you are accepted, you will receive a letter notifying you of your provider account number. After 90 days, if you haven’t heard from us, please email us at PACMail@Lni.wa.gov.
