Section 1115 Demonstration Waiver Preconception Care (PCC)
SCDHHS will submit a Section 1115 Demonstration Waiver application to the Centers for Medicare & Medicaid Services (CMS)to implement a preconception care (PCC) model to improve health, reduce or remove risk factors and identify disease as early as possible in women and men during their reproductive years. The demonstration project, or PCC model, requires the coordination of family planning and reproductive health care with primary care services and chronic disease management for both full- and limited-benefit Medicaid beneficiaries.
