If you have received treatment through a local management entity (LME) and you would like to obtain your medical records, contact your LME directly.

• Each LME has a person who is in charge of medical records requests and questions.
• This individual can help you get records from the LME
• This individual will also be able to help you even if the provider is no longer in business
