Drivers License Division

The Drivers License section is responsible for a number of programs. We issue new and renewal licenses for drivers of all motor vehicles, State of Hawai'i Identification Cards (SID) new, renewals and duplicates. We also issue disabled persons placards for parking, and taxi operator permits. This department is also responsible for the Commercial Driver License Program's testing and licensing.

Requirements to Obtain a Driver License or State Identification Card, Out of State Transfers and new Permits

  • Proof of Social Security Number: SS Card, W-2 or 1099 Tax form, or pay stub with the complete SS#
  • Legal Presence Document: Valid U.S. Passport, U.S. Passport Card or U.S. Birth Certificate, Foreign Passport with U.S. Immigration Admittance Documents, Naturalization Certificate, Permanent Residence Card, Foreign Passport with current admitance stamp or I-94, etc.  (Original or Certified Copy Only)
  • Name Change: Name change trail must be shown from birth name to current name. Marriage Certificate, Legal Name Change Document, Divorce Decree, etc. (Original or Certified Copy Only).
  • Proof of Principle Address: Two (2) documents containing applicants residential address.
  • Note:  Additional documents may be required if there is a difference in names on the documents submitted. Documents presented must be valid originals or certified copies.  Photo copies or faxes are not acceptable as proof.

NEWS RELEASE

Beginning May 1, 2014, Applicants will be required to present proof of principal residence in Hawaii. Principal residence is defined as the location where a person currently resides even if the residence location is temporary. Any two of the following documents (original or copy) containing the applicant’s principal residence will be acceptable proof:
1. A current valid Hawaii driver’s license;
2. Vehicle registration or title;
3. A current voter registration card
4. Mail from a government or medical entity;
5. Utility bill with applicant's name and address;
6. Checking or savings account statement;
7. Payroll check or check stub issued by an employer;
8. Current mortgage account or proof of home ownership;
9. Residential rental or time share contract;
10. United States income tax return, W-2 form or 1099 SSA benefits;
11. Hawaii income tax or W-2 form;
12. Receipt for personal property taxes paid to a county within the State of Hawaii;
13. Medical card issued by a health insurance agency with principal residence address printed on it;
14. Current property tax assessment bill or statement;
15. A stamped department of taxation form A-6, application for tax clearance;
16. Applicants documenting enrollment in a State or Federal address confidentiality program which allows an applicant to obtain and use alternative addresses may use an alternative address on the card but must provide the applicant’s permanent address for file purposes;
17. In areas where a number and street name have not been assigned for U.S. mail delivery, an address convention used by the U.S. Postal Service is acceptable (P.O Box or R/R#)
18. Affidavit indicating that the applicant currently resides with the affiant, provided the affiant's address can be verified and the affidavit is notarized within two months of the application date; or
19. Other documents the examiner of drivers accepts as proof of principal residence in the State of Hawaii.

 

ACCEPTABLE DOCUMENTS FOR VETERAN DESIGNATION Effective October 1, 2014

 

NOTE: Applicable to any person who served in any of the uniformed services of the United States and was discharged under conditions other than dishonorable.

·         DD Form 214 (Certificate of Release or Discharge from Active Duty) The Defense Department issues to each veteran a DD-214, identifying the Veteran’s condition of discharge – honorable, general, other than honorable, dishonorable or bad conduct. Before January 1, 1950, several similar forms were used by the military services, including the WD AGO 53, WD AGO 55, WD AGO 53-55, NAVPERS 553, NAVMC 78PD, and the NAVCG 553.

o   WD AGO 53-55 (Enlisted Record and Report of Separation Honorable Discharge)

o   NAVPERS 553 (Notice of Separation from U.S. Naval Service)

o   NAVMC 78PD (U.S. Marine Corps Report of Separation)

o   NAVCG 553 (Notice of Separation from U.S. Coast Guard)

 

o   NGB Form 22 (National Guard Report of Separation from Active Duty)

  •     DD Form 217 (Certificate of Service)

·         Official Military Orders   

·         Discharge Certificate

·         Military Retired ID Card (DD Form 2 (Retired)

·         Veterans Health ID Card (VHIC)

·         Verification letter issued by the Veterans Administration

·         Verification letter issued by the Office of Veteran Services

 

If you’re unable to produce any of the proper documentation, may contact the State of Hawaii Office of Veterans Services who will research their case and provide a Verification Letter if they qualify for the veteran status.
