How do we submit our application?

Applications should be submitted to your respective Grants Management Specialist (GMS) via email by March 12, 2018 11:59 PM EST. The MIS templates should be submitted in their original Excel format. When submitting your application, please send a courtesy copy to your project officer.

What funding amount should we use to prepare our application ?

Funding level memos were posted in GrantSolutions under “Grant Notes”; amounts are consistent with your year 4 award.

CDC is still operating under a Continuing Resolution. Therefore, we do not currently know the final budget numbers.

Is the Annual Progress Report (APR) similar to what you want to see in the Evaluation Report?

The APR is a report of your work from February 1, 2017 to January 31, 2018. Your Evaluation Report should follow the evaluation plan that was submitted with the initial application at the beginning of the cooperative agreement. It should cover the first four years of the cooperative agreement.

The deadline for the Evaluation Report has been extended to July 31, 2018; however, awardees may still submit it with the continuation application on or before the deadline. If assistance is needed, please contact your Project Officer.

Is the "required travel" to Atlanta?

Yes. Budgets should be prepared for one staff member to attend a meeting in Atlanta.

Do all of the extra documents that are now on GrantSolutions get printed out and emailed with the rest of the package?

Yes. All required forms and certifications provided on GrantSolutions should be emailed to your Grants Management Specialist. The application packet should consist of the following:

  • SF424 Application for Federal Domestic Assistance version 2
  • SF-424A Budget Information-Non-Construction
  • SF424B Assurances-Non-Construction
  • Indirect Cost Rate Agreement
  • Certifications
  • SF-LLL Disclosure of Lobbying Activities
  • MIS templates provided by your Project Officer for the Budget Justification, Annual Progress Report, and Work plan

Is it a requirement for state Quitlines to screen callers for persons in the military and veterans?

No. Although not required, awardees are encouraged to screen callers for active duty and veteran status.

Are awardees required to make referrals to smoking cessation Quitlines in the Department of Defense and/or Veterans Affairs?

No. Although not required, awardees are encouraged to refer active duty military callers to the smoking cessation Quitline run by the Department of Defense. Veterans who receive their health care through the VA should be referred to the Quit VET Quitline.
