State and Local Public Health Actions Program

Through CDC’s Division for Heart Disease and Stroke Prevention, the State and Local Public Health Actions and Good Health and Wellness in Indian Country cooperative agreements fund state- and city-wide initiatives and tribes to prevent, manage, and reduce the risk factors associated with heart disease and stroke. The funding for heart disease and stroke prevention is part of four CDC programs that work together to support chronic disease prevention initiatives nationwide.
