Forms.IN.gov provides citizens and employees of the State of Indiana a common access point to State forms. Managed by the Indiana Archives and Records Administration, the Catalog contains electronic versions of relevant and current forms, allowing citizens to more easily conduct business with the State of Indiana.
Not all State forms are available on the Catalog. Some forms are only available directly from the State agency which publishes them, at that agency's request.
The maintainers of Forms.IN.gov cannot provide assistance with filling out or submitting State forms, or determining which particular form you might need. For questions about a specific form, please contact the agency which requires you to submit that form. (There is frequently contact information on the form itself; if none is provided or you don't know which form to use, you can also contact the Forms Coordinator for the appropriate State agency.)
