Request Your Medical Records

Our Release of Information staff will be happy to assist you with requests for your medical records. We can assist you with the following:

  • Access to your medical records
  • Obtaining copies of your medical records
  • Requests to amend or correct your medical records
  • An accounting or list of disclosures of your health information
  • Authentication for My-Health-e-Vet

We are available Monday - Friday 8:00 am - 4:30 pm for your convenience.

How to Request Information

To request a medical record, please use the appropriate form listed in the column to the right, complete the requested information, sign the form, and mail it or hand carry it to the following address
