Nomination Process

Nominators should submit a completed nomination form to their state or territorial immunization program, by clicking the “Nominate a Champion” button on this page.

You can nominate yourself or another provider. Nominations will be accepted from all 50 U.S. states, 8 U.S. Territories and Freely Associated States, and the District of Columbia. The nominee must operate within a single HHS regionExternal.

Nomination Deadline

The suggested deadline for nominations to state or territorial immunization programs is August 10, 2018. However, nominators should contact their immunization program to find out if they have a different deadline. See contact information for State and Territorial Immunization Programs to submit nominations.

State and territorial immunization programs should submit their nomination to CDC at PreteenVaccines@cdc.gov by September 14, 2018. Please note that only one nomination will be accepted from all 50 U.S. states, 8 U.S. Territories and Freely Associated States, and the District of Columbia.

You must include the following in your nomination submission:

  • Completed nomination form
  • Image or photograph of the nominee, which may be used for promotional purposes

Award Recognition

The 2018 award recipients will be announced in the fall of 2018. Champions will be featured on CDC’s website and in the #PreteenVaxNews e-newsletter. Champions will also receive a congratulatory letter from AACI, ACS, and CDC, an HPV Vaccine Is Cancer Prevention Champion plaque, and a digital web badge for their websites.
