Medicaid Central Intake Unit (CIU)	866-940-8963	The CIU accepts referrals and applications for Nursing Home or Community-based Long-Term Care Medicaid.
Medicaid Health Benefits Manager "HP Enterprise Services"	800-996-9969	The Medicaid Health Benefits Manager assists you with enrolling in a Managed Care Organization (MCO) and understanding your benefits and prescriptions.
Chronic Renal Disease Program	302-424-7180	Ask for "Chronic Renal Disease Program." The Chronic Renal Disease Program (CRDP) provides assistance to Delaware residents diagnosed with End Stage Renal Disease (ESRD).
Delaware Healthy Children Program	800-996-9969	The Delaware Healthy Children Program is a low cost health insurance program for Delaware's uninsured children up to age 19.
Fraud Reporting	800-372-2022	Fraud Reporting receives referrals of suspected fraud, waste or abuse of Medicaid benefits. You may also email to SURreferrals@delaware.gov
Provider Services "HP Enterprise Services"	800-999-3371	Provider Relations assists Delaware Medical Assistance Program (DMAP) Providers with enrolling in the Medicaid program, receiving and responding to provider inquires, verifying client eligibility and aiding in electronic claims submission.
Transportation Services "Logisticare"	800-412-3778	“Logisticare” provides non-emergency transportation to and from a medical service.

Delaware Prescription Assistance Program	1-800-996-9969	Help paying for prescription medications for disabled and/or elderly individuals who cannot afford the full cost of filling their doctor’s prescriptions.
