Update for January 13, 2021: At this time, all appointments for COVID-19 vaccine have been filled. Today, the Florida Department of Health in Polk County, in partnership with Polk County Board of County Commissioners, launched Polk County’s COVID-19 vaccine registration portal. This portal is an online reservation system for those who meet the requirements in the governor’s executive order and wish to receive the vaccine as it’s available. This includes individuals 65 years of age and older and healthcare workers who provide direct patient care.
The portal can be found by visiting Polk County Government’s website or by clicking the link below. Users can create an account and upload their personal information for a future appointment to receive the COVID-19 vaccine. Even though the portal is live, individuals may still call the COVID-19 Vaccine Hotline to register their information if they prefer to speak to someone over the phone. The vaccine hotline number is 863-298-7500. Due to heavy call volumes to the hotline, callers may experience a busy signal or receive an automated message. We ask for patience as we continue to work through this process.
To date, more than 13,700 individuals in Polk County have received the COVID-19 vaccine so far.
