Click here to submit your completed application and paperwork to DSB
Your application will not be evaluated without the following:
– Billing statements or invoices for unavoidable expenses to support amount of funding request
– Proof of payment over the past 12 months for expenses for which funding is requested (account statement or letter from the relevant vendor/creditor)
– Balance sheet and income/P&L statement for 2019 (2019 tax returns may be used as an alternative, must include accounting of assets and liabilities)
– Copy of Delaware business license
