Correcting or Amending Vital Records
ā**NOTICE REGARDING REAL ID**The deadline to obtain a Real ID has been extended to October 1, 2021. The REAL ID Act is a new federal law regarding driverās licenses that may affect those individuals whose vital records do not match their driverās licenses. To prevent delays when applying for your driverās license, you are encouraged to review your birth and/or marriage to make sure your name, sex, and date of birth are listed correctly. In an effort to assist, CDPH has provided a list of common scenarios that require an amendment to the personās birth and/or marriage certificate. Requests to correct or amend vital records require:
- A completed amendment application
- Supporting documentation
- Appropriate fee(s)
- A notarized sworn statement completed by an authorized person ā Only if requesting an authorized certified copy
NOTE: With the exception of Court Report of Adoption (VS 44) forms that are submitted by court clerks, if a notarized sworn statement is not received, an informational certified copy may be issued. Please visit the
