If your medical cannabis registry identification card was stolen, you should report it to the police immediately.
To request a new registry card, please complete the Replacement of a Lost or Stolen Medical Cannabis Registry Identification Card form. There is a $25 fee for the replacement card. The form should be mailed with a check or money order to:

Once your request is received with payment, it will be processed and a new card will be printed and mailed to you.
Please note, you cannot access a medical cannabis dispensary until you receive your new registry card.
