Interim Draft COVID-19 Vaccination Plan for New Hampshire

NH is committed to a safe and effective vaccine that will protect against COVID-19. We are prepared to receive the initial batches of FDA-approved COVID-19 vaccine in mid-December 2020. The vaccine will be distributed in a phased approach and the initial allocation will be used to vaccinate at-risk health workers, residents of long-term care facilities, and first responders.

New Hampshire Interim Draft COVID-19 Vaccination Plan

This draft plan is a living document and will be revised as necessary as new information and guidance is known about the vaccine candidates. NH is taking steps to ensure we are ready to receive a vaccine and have a plan that allows for equitable access for every New Hampshire resident.

New Hampshire COVID-19 Vaccine Allocation for All Phases

Phase 1a

At-risk health workers, residents of long-term care facilities and first responders.

*Additional phases will be added as future vaccine allocation is determined.

Phase 1b

Persons ≥65, the medically vulnerable, intellectual and developmental disability residential facilities, correctional facility staff, and remaining health workers and first responders.

COVID-19 Vaccination Provider Agreement

As additional COVID-19 vaccine becomes available for Phase 1b individuals and beyond, NH DHHS will distribute COVID-19 vaccines to provider offices, urgent care, pharmacies and other local health care facilities that have the capacity to offer vaccination to patients. To receive/administer COVID-19 vaccine, constituent products, and ancillary supplies, health care organizations that want to receive COVID-19 vaccine must enroll in the federal COVID-19 Vaccination Program coordinated through the New Hampshire Immunization Program. Enrolled COVID-19 vaccination providers must be credentialed/licensed in the jurisdiction where vaccination takes place and sign and agree to the conditions in the CDC COVID-19 Vaccination Program Provider Agreements. These conditions are detailed in the agreement itself. Note that NH DHHS will only be able to begin providing vaccine to these entities when vaccine supply allows.

In order to enroll as a COVID-19 vaccination provider, download the Provider Agreement pdf file complete the form electronically, being careful to follow the instructions, and email to C19Enrollment@dhhs.nh.gov.
