Florida has many sites providing COVID-19 vaccine. Use the vaccine finder to find a site near you. Supplies are limited and appointments may be required. At this time, vaccine is available for:  Persons 65 years of age and older.  Health care personnel with direct patient contact.  Residents and staff of long-term care facilities.   

Update for January 21, 2021: The Florida Department of Health in Polk County continues to work diligently to protect the health of the residents in Polk County against COVID-19. This week, DOH-Polk is working with community vaccination partners including Lakeland Regional Health and BayCare, to assist with COVID-19 vaccinations across the county. These partners will be assisting DOH-Polk in vaccinating those who are on Polk’s vaccine registration list. The registration list includes individuals who have called into Polk County’s COVID-19 Vaccine Hotline or registered through the online portal. Vaccines will be given by appointment ONLY. As more vaccine becomes available, DOH-Polk hopes to expand its vaccination partners to different entities including pharmacies, drug stores and primary care providers.

DOH-Polk, in partnership with Polk County Board of County Commissioners, launched Polk County’s COVID-19 vaccine registration portal on Thursday, January 14th. This portal is an online reservation system for those who meet the requirements in the governor’s executive order. Those who are 65 years old and older and who wish to receive the COVID-19 may register through the portal. Users can create an account and upload their personal information to be notified about a future appointment as vaccine is available.

Polk County’s COVID-19 Vaccine Hotline also remains open. Those who prefer to register by phone, can still call (863) 298-7500 for assistance. The center is open 8 a.m. to 5 p.m. Monday through Friday. Additional weekend hours have also been added for 8 a.m. to 2 p.m. Saturday and Sunday.

As of January 19th, 2021, more than 19,200 individuals have been vaccinated across Polk County.
