Help with a Federal Agency

One of the most important things I do as a U.S. Representative is help people navigate the federal government. Though I cannot override the decisions made by a federal agency, I can often intervene on a person's behalf to answer questions, find solutions, or just cut through the red tape. Below you will find a list of agencies that I can help you with. However, regardless of which agency you need help with, I need a completed privacy release form (linked below). Once you complete this form, we will contact you to fill out a privacy authorization form to comply with the provisions of the Privacy Act.

Residents of the 2nd Congressional District of Utah can contact me for assistance in dealing with Federal agencies. You can find your Congressional representative by inputting your address here. In order to better serve you, please fill out the forms below and mail them to my Bountiful City Office. As always, feel free to call my Bountiful City Office at 801-364-5550 if you have any specific questions.
