HELP WITH A FEDERAL AGENCY

If you cannot get an answer from a federal agency in a timely fashion, or if you feel you have been treated unfairly, Congressman Raskin's staff may be able to help resolve a problem or get you the information you need. While we cannot guarantee you a favorable outcome, we will do our best to help you receive a fair and timely response to your problem.

To receive assistance from our office, you must first fill out, sign and return a Privacy Release Form. Please choose one of the methods below for submitting a completed Privacy Release Form. 

Please note: The Privacy Act of 1974 requires that written consent be obtained from a constituent before information from federal agency records can be disclosed. In order for Representative Raskin's staff to conduct an inquiry on your behalf, you must sign and submit a privacy release form.

ONLINE FORM

Click here to use the online portal. You can digitally sign and submit the appropriate form, along with any supporting documents.

MAIL FORM

Print, fill out, and sign the privacy release form. You should mail it to the District Office at 51 Monroe St, Suite 503, Rockville, MD 20850.
