Statutorily Required Reports

The Wisconsin Department of Health Services (DHS) is statutorily required to provide one-time, quarterly, annual, and biennial reports on behalf of the agency. DHS is required to provide these reports and information to the legislature and governor.

Wisconsin Stat. § 20.907(1m)

Reporting. State agencies shall, by December 1 annually, submit a report to the joint committee on finance and the department of administration on expenditures made by the agency during the preceding fiscal year from nonfederal funds received as gifts, grants, bequests or devises.

Wisconsin Emergency Medical Services State Plan

Wisconsin Stat. § 256.08

(a) By December 31, 1995, the department shall prepare a state emergency medical services plan. The plan shall include an identification of priorities for changes in the state emergency medical services system for the two years following preparation of the plan. In preparing the plan, the department shall review all statutes and rules that relate to emergency medical services and recommend in the plan any changes in those statutes and rules that the department considers appropriate. After initial preparation of the plan, the department shall keep the plan current and shall reorder priorities for changes in the state emergency medical services system, based on determinations of the board.

Annual Report to the Legislature on the Wisconsin Health Information Organization (WHIO)

Wisconsin Stat. § 153.05(2s)

(2s) Annually, the department of health services and the department of employee trust funds shall jointly prepare and submit under s. 13.172 (3) to standing committees of the legislature with jurisdiction over health issues a report on the activities of the data organization under this chapter.

Report on Medication-Assisted Treatment in Prisons and Jails

2019 Act 119

(2) Proposal for availability of medication-assisted treatment; report. The department of health services shall, using the results of the study under sub. (1) and after consulting with the department of corrections, develop a proposal to implement, or identify county officials to implement, a pilot project to make available in at least one prison or county jail all medications for medication-assisted treatment for opioid use disorder that are approved by the federal food and drug administration.

Community Options Program (COP) Waiver and Report to the Legislature

Wisconsin Stat. § 46.27(11g)

Beginning January 1, 1997, and every January 1 thereafter, the department shall submit a report to the joint committee on finance and to the appropriate standing committees under s. 13.172 (3), summarizing the data collected for the state and for individual counties under the program in the calendar year ending immediately before the preceding calendar year.

Cost to Certain Prevailing Parties Report

Wisconsin Stat. § 814.245(10)

Each state agency that is ordered to pay costs under this section or that recovers costs under sub. (11) shall report annually, as soon as is practicable after June 30, to the presiding officer of each house of the legislature the number, nature and amounts awarded, the claims involved in the action in which the costs were incurred, the costs recovered under sub. (11) and any other relevant information to aid the legislature in evaluating the effect of this section.

Wisconsin Hospital Association Information Center Reports

Wisconsin Stat. § 153.05(2m)(c)

(c)  By April 1, annually, the secretary of health and family services shall submit to the chief clerk of each house of the legislature for distribution to the legislature under Wis. Stat. § 13.172(2), a report concerning the content and number of reports and currency of information and reports generated in the previous calendar year by the entity under contract under § 153.05 (2m).

Hunger Report

Wisconsin Stat. §§ 49.76(4) and (5)

(4) Develop an annual plan that documents areas of hunger and populations experiencing hunger within this state and that recommends strategies and state and federal policy changes to address hunger in these areas and populations.
(5) Submit, by December 31 annually, the plan developed under sub. (4) to the governor, superintendent of public instruction and the appropriate standing committees under s. 13.172 (3).​

Report on the Success of the Statewide Immunization Program

Class A Violations for Nursing Homes and FDDS

Wisconsin Stat. § 50.04(5)(fr), Wis. Act. 237

As defined by s. 50.04(4)(b)(1), a Class A violation is “a violation of this subchapter or of the rules promulgated thereunder which creates a condition or occurrence relating to the operation and maintenance of a nursing home presenting a substantial probability that death or serious mental or physical harm to a resident will result therefrom.”

Medicaid Prior Authorization Policy for Buprenorphine-Containing Products for Medication-Assisted Treatment

Wisconsin Stat. § 49.45(29z)

Buprenorphine prior authorization review. The department shall review its prior authorization policy on buprenorphine-containing products provided to Medical Assistance program recipients. On the first day of the 7th month beginning after the effective date of this subsection .... [LRB inserts date], and every 6 months thereafter, the department shall submit to the standing committees of the legislature with jurisdiction over health under s. 13.172 (3) a report describing the department's findings on the prior authorization policy on buprenorphine-containing products and its progress on eliminating prior authorization requirements for buprenorphine-containing products in populations where removal of prior authorization is appropriate.

Section 1. 51.42 (7) (d) of the statutes is created to read:
51.42 (7) (d) By January 1, 2015, and by January 1 of each odd-numbered year thereafter, the department shall submit to the legislature under s. 13.172 (2) a report that describes mental health services and programs provided by counties and regions comprised of multiple counties.

Protecting Wisconsin: Health Emergency Preparedness

Wisconsin Stat. 250.03(3)(b)

Biennially, after first consulting with the adjutant general, local health departments, health care providers, as defined in s. 146.81 (1) (a) to (p), and law enforcement agencies, as defined in s. 165.77 (1) (b), the department shall submit to the legislature under s. 13.172 (2) and to the governor a report on the preparedness of the public health system to address public health emergencies.

Tobacco Use Cessation: Annual Report to the Governor and the Legislature

(4) Reports. Rehabilitation Requests Report

Wisconsin Stat. § 50.065(5g)

(5g) Beginning on January 1, 1999, and annually thereafter, the department shall submit a report to the legislature under Wis. Stat. § 13.172 (2) that specifies the number of persons in the previous year who have requested to demonstrate to the department that they have been rehabilitated under sub. (5), the number of persons who successfully demonstrated that they have been rehabilitated under sub. (5) and the reasons for the success or failure of a person who has attempted to demonstrate that he or she has been rehabilitated.
