ForwardHealth: Resources for Community Partners

The Wisconsin Department of Health Services (DHS) is seeking applications from community-based organizations, health care agencies, and community groups interested in engaging with patients, clients, or individuals they serve to obtain feedback about their DHS services and programs. The data gathered will be used to inform decision-making for program and service improvements for the grantee agency as well as for the Division of Medicaid Services.

Community-based agencies and groups have a long history of engagement and trust built with the populations they serve. The request for applications enables DHS to compensate organizations as trusted messengers to gain feedback from the people DHS programs serve.

Policy updates

We host biweekly policy calls in our stakeholder call series. These calls are not recorded, but they inform the community of updates to our eligibility policy and changes to rules that affect programs like FoodShare and Medicaid.
