Every applicant for a non-immigrant visa, including children, must present a separate, completed Form DS-156. All male non-immigrant visa applicants between the ages of 16 and 45 must present a completed Form DS-157 in addition to Form DS-156. All students applying for F1, M1 and J1 visas must present a completed Form DS-158. Application forms must be completed in English and must be signed by each individual applicant (parents may sign for children under fourteen years old). INCOMPLETE APPLICATIONS WILL NOT BE ACCEPTED. If someone other than the applicant completes Form DS-156, the person completing the form must sign his/her name at the bottom of item #35 on Form DS-156. Applicants are responsible for the accuracy of the information provided in the application forms.
