AUTHORITY: Sections 1302, 3301, and 7201 of Title 5 U.S. Code. This information will be used to update your resume for employment purposes. Collection of your Social Security Number (SSN) is authorized by Executive Order 9397. Furnishing the information on this form, including your SSN is voluntary, but failure to do so will result in non-referral.
We must have your SSN to uniquely identify your record because other persons may have the same name and birthdate. As allowed by Law and Executive Order, we may also use your SSN to request information about you from employers, schools, banks, and others who know you. The information we collect using your SSN will be used for employment purposes.
