6. Keeping Track of Your Child's Health

The charts in this section can make it easier to keep track of your child's health. Ask your child's doctor or nurse how often he or she needs each type of care and when. Then write down the information in the charts here. Also, write down the date and results of the care your child gets or ask your child's doctor to write down this information.

Download these charts and bring them with you each time your child sees a doctor or nurse. That way, you can update them right away.
