The Symposium is sponsored by the Department of Health and Human Services (HHS) Office on Disability with the HHS Centers for Medicare and Medicaid Services (CMS), Fannie Mae Corporation, NCB Development Corporation, and the Department of Housing and Urban Development (HUD).

Teleconference Information:

Live webcast capacity provides opportunity for those with this electronic capability to participate. Both the resulting captioned webcast and video will be available for one year after the Symposium. Upon request, a video or CD will be made available for organizations that can and cannot participate in person or by live webcast but personally do not have the ability to electronically download the webcast.

Meeting Location:

The meeting is at the Department of Health and Human Services, Hubert H. Humphrey Building, 200 Independence Avenue, S.W., Washington, D.C. Due to limited conference room capacity (up to 50 constituents), preference for in-person participation is for those without webcast capacity. To participate by live webcast, viewers need to download the latest free version of Realplayer software from www.real.com.

Meeting Schedule:

9:00 AM to 4:30 PM with in-person registration beginning at 8:00 AM. Lunch will be on your own. The HHS cafeteria is located on the eighth floor of the Hubert H. Humphrey Building.

Meeting Registration:

All participants must register by Friday, June 18, 2004 to Ryan Faden by e-mail at rfaden@aphsa.org, or by fax at 202.682.3706. Upon receipt of the registration form, a confirmation email (telephone call for those who do not have a computer) will be sent including information on the Web access. The registration form is attached.
