This document is a “how to” guide for planning and implementing evaluation activities. The manual reflects the priorities of the Centers for Disease Control and Prevention for program planning, monitoring and evaluation under the Supported States Infrastructure Development Program.

The purpose of this manual is to assist state programs in planning, design, implementation, and use of practical and increasingly comprehensive evaluation of oral health promotion and disease prevention efforts. The strategy presented in this manual will aid those responsible for program planning and evaluation activities to demonstrate accountability to diverse stakeholders. This is a work in progress, as additional sections are completed they will be placed on this page.
