Requirements for Tracking Soldier Pre- and post-deployment Health Status through SRP/PDP Processing

An important and powerful component of the Army Force Health Protection (FHP) programs is a simple-to-complete pre and post deployment survey for each deploying/redeploying soldier. DoD requires soldiers to complete at DD Form 2795 (Pre-Deployment Health Assessement) prior to deploying and a DD Form 2796 (Post-Deployment Health Assessment) prior to redeploying. Currently the U.S. Army Medical Suveillance Activity (AMSA) is tracking this process through Soldier Readiness Program (SRP) Rosters used in CONUS and Pre-Deployment Process (PDP) used in OCONUS. These rosters are submitted by the SRP/PDP sites or redeployment sites throughout CONUS and OCONUS. In order to better track this information and provide it in a more useable format, the SRP roster that is currently submitted with the daily SITREPS to MEDCOM EOC has been updated. The following files provide the information, instructions and examples on how these files should be submitted to MEDCOM with the daily SITREPS.
