Former members of the Regular Air Force,  Air Force Reserve, and Air National Guard.

If the former member is deceased or incompetent, surviving spouse, next of kin, or a legal representative may apply with supporting documentation, e.g., marriage license, death certificate, or power of attorney.

Submit DD Form 293, Application for the Review of Discharge or Dismissal from the Armed Forces of the United States. The form, DOD Directive and Instructions are located under 'Forms and Publications'

Discuss concerns/obtain assistance from a Veterans' Service Organizations
