Forms and Instructions

To retrieve an item please follow these steps:

  • Select a file format. For additional information about a format click on the format name below. (PDF format allows you to view your form electronically on most computers. The freely available Adobe Acrobat reader is required to view and print PDF files.)
  • Select the file(s) you wish to receive. To select multiple items, hold the control button down while selecting.
  • Press the 'Review Selected Files' button. A results page will be displayed with links to the file(s) you requested.
  • Select the file title to retrieve.

Note: we will begin merging fill-in forms with our main Forms and Instructions list on December 1, 2003. Fill-in forms allow you to enter information while the form is displayed by an Adobe Acrobat product and then print the completed form for your records and for filing with the IRS.

Please review the Fill-in Forms Instructions for software requirements and detailed instructions on usage. Caution: the Acrobat Reader does not allow you to save your fill-in form to disk.
