Fulfillment of DoD Mandatory Training Requirement
This form, when accompanied by a self-assessment form, is used by the supervisory chain to determine whether the member demonstrates the prescribed competencies sufficiently to forego attendance at the specified course.

DoD Form 2602, Sep 91

Critical Acquisition Position Rotation Review Summary
This form is used to document the rotation review of personnel incumbering a Critical Acquisition Position (CAP) five years. The initial review is conducted on the anniversary of the appointment to the position and annually on August 31 each year thereafter.

DON Travel Request/Authorization for Acquisition Training
This form is used by members of the acquisition workforce to obtain funding for travel required for attendance at mandatory acquisition training.

DON Acquisition Training Registration Request
This form can be used by civilian and military personnel who do not have access to the Internet to register for acquisition training required by the Defense Acquisition Workforce Improvement Act (DAWIA).
