Instructions for Completing DTIC's Registration Form

1) See "Easy
Step" Instructions for a brief outline of registration

2)A Separate DD Form 1540 must be
completed for each contract.
3) Potential Contractors must
first sign--up the Potential Defense Contractor Program. See Part
2 Section 2.
4) For access to export-controlled
data, you will also need to complete the DD
Form 2345, Militarily Critical Technical Data Agreement.

Section
I
(General Information) |
Section I solicits
specific information about your organization and the individual requesting
access. This individual must be fully eligible to receive the level
of information indicated in Item #11, Type of Access Desired, on the
DD Form 1540. Supporting documentation must be provided. (See Item
#11.)
Items #1-3. Complete in full.
Item #4. Attention. Identify the individual who is registering
with DTIC® and is authorized to receive products and services.
This individual must be authorized to receive the level of information
indicated on the DD Form 1540 in Block 11, Type of Access Desired.
The Facility Security Officer must provide documentation to support
this person's eligibility to receive all of the information requested.
Note: The Social Security Number is not needed for contractors.
If it is provided it will not be retained in DTIC's files.
Items #5-6. Complete in full.
Item #7. Internet E-Mail Address. Include e-mail address of
individual identified in Item #4.
Items #8 a & b. Prime Contract Number and Expiration Date.
Include the contract number and expiration date of the contract you
would like to register with DTIC.
Note: Separate DD Form 1540 must be completed for each contract.
Item #9. Current or Former
User. Are you a current or former DTIC user? If you answer "yes,"
enter your current or past DTIC User Code in the space provided.
Item #10. Militarily Critical Technical Data Agreement Certification
Number. If you do not have this number and you are requesting access
to export-controlled data, you will need to complete the DD Form
2345. See page 12 for further details.
Item #11. Type of Access Requested. Place an X in the appropriate
boxes.
The Facility Security Officer must provide documentation showing
that the individual on the attention line is eligible to receive
all requested classifications of information. He/she must provide
a transmittal letter with attachments. The subject of the transmittal
should be "Transmittal of Application for Registration w/Supporting
Documentation". The transmittal should state that the organization
supporting NATO Control and accountability is assigned to the Control
Point.
The letter should provide the person's name and state that he/she
is a U.S. citizen who has a need -to-know and has been briefed
and granted access to classified information up thru Confidential/Secret;
to include NATO, CNWDI, and Restricted Data, if appropriate. The
letter should include the level of the requestor's classified storage
capability and classified mailing address. The letter must be signed
by the Facility Security Officer.
Note: Non-DoD contractors requesting
access to Restricted Data and/or CNWDI must have the Department
of Energy (DOE) notify DTIC in writing that the individual listed
in Section I is authorized to receive such data. Refer to Appendix
A for the DOE address.

NOTE: A sub-contractor is
no longer required to obtain the prime contractor's approval. Send
your DD Form 1540 directly to your U.S. government approving official.
After the approving official signs, the DD form 1540 needs to be
forwarded to the DTIC Registration Team for processing.

Section
IV (Government
Approving Official) |
Section IV is to
be completed by all contractors, subcontractors and potential contractors
and must be signed by their Government approving official.
Items #17-18. Complete in full.
Non-DoD sponsored contractors should refer to Appendix A, "Non-DoD
Offices Designated to Approve DD Form 1540," to locate the appropriate
approving official.
The approving official for DoD-sponsored contractors is the
Contracting Officer, Contracting Officer's Technical Representative,
Program Manager, Program Engineer,Contract Monitor or other authorized
designee.
Note: Contractors may have
the Administrative Contracting Officer (ACO) certify the DD Form
1540, provided that the Procurement Contracting Officer (PCO) has
notified DTIC in writing that the ACO may do so. Otherwise, the
PCO must sign the DD Form 1540.
Item #19. Completed by the
Government approving official. His/her signature is required. In
the example below NASA Security Officers are the authorized designees
to sign the DD Form 1540. NASA is shown here only as an example
to show how a non-DoD contractor would complete Section IV.

V
Subject Fields and Groups) |
Section V is to
be completed if your organization is requesting access to classified
information (Confidential/Secret) and/or Restricted Data, CNWDI OR
NATO Information.
If you are requesting access to an entire subject field, place an
X in the first block of that field (indicating ALL). However, if less
than the entire subject field is required, place an X in the block
for each subject group required.

Forwarding
the DD Form 1540 |
Forward the DD Form 1540 to
the approving official for signature. The approving official may
mail or fax the form to the address and fax numbers as follows:

Completing
the
DD Form 2345 |
DD Form 2345, Militarily
Critical Technical Data Agreement, must be completed if access to
export-controlled data is requested and you do not have a certification
number from the Defense Logistics Information Service (DLIS).
Complete blocks as instructed on the form and forward to DLIS for
approval at the address shown on the form. The data custodian on the
DD Form 2345 and the individual on DD Form 1540 must be the same.
Certification is valid for 5 years from the date of DLIS' signature,
provided the information does not change. When you receive the certification
from DLIS, mail or fax a copy of the approved DD Form 2345 to the
address or fax numbers above.
If you have received a certification number from DLIS, DTIC can verify
your certification number in the Joint Certification Program via a
DLIS database.

Facility
Clearance |
Facility Clearance
Certification from the Defense Security Service (DSS) authorizes access
to classified data. If your organization is not certified, your organization
must arrange for a DSS representative to survey the facility to be
used to store classified data. Refer to Appendix B to locate appropriate
DSS points-of-contact in your area.

Please state that you are requesting
unclassified access only. (To receive classified access, you must
still complete a DD
Form 1540.
Contractors who already have access to export controlled information
must use the point of contact and business address reflected on
the DD Form 2345 (Militarily Critical Technical Data Agreement).

Receipt
of DTIC User Code |
Once your registration
is processed by DTIC, you will receive a welcoming email letter via
email. This letter will include your DTIC user code. You will also
receive a package via regular mail that will include a copy of your
processed DD Form 1540 and other information about DTIC.

Ordering
from DTIC |
Before you order products
or services from DTIC you must inform the Registration Team of how
you intend to pay. We recommend that you use one of the following
major credit cards: VISA, MasterCard, or American Express.
You are now ready to order DTIC's products and services! You must
apply for receipt of specific products or for access to online services.
Remember to always have your DTIC User Code ready when placing orders.
