Accessing Forms

To access TTB forms you must:

• Select the form number from the classifications below
• Download and print a hard copy
• Have the latest version of Adobe Reader or download it here

Note - PDF files may not be viewable through the AOL browser.

Three ways to order a form:

• In writing - National Revenue Center, 550 Main Street, Suite 8002, Cincinnati, Ohio 45202
• By telephone - 1-877-882-3277
• By e-mail - ttbformsrequest@ttb.gov

Note - Your order must include your name, delivery address, telephone number, TTB form number, and the quantity of each form.

Submitting Forms

Forms must be:

• Tax return forms should be submitted according to the instructions on the form.
• Other forms must be submitted to the National Revenue Center, 550 Main Street, Suite 8002, Cincinnati, Ohio 45202, complete with signature and date.
  

Commenting on Forms

You may comment on any TTB form by writing to the Alcohol and Tobacco Tax and Trade Bureau, Regulations and Procedures Division, 1310 G Street, NW, Room 200-East, Washington, DC 20220 or by e-mail at formcomments@ttb.gov. You may also examine the Federal Register notices as required by the Paperwork Reduction Act of 1995.
