Medical Evaluation of Lung Function

For Former Ore Processing Workers,
Stauffer Chemical Company, Tarpon Springs Plant, Florida

What?
The Agency for Toxic Substances and Disease Registry is paying for this medical evaluation. The medical evaluation includes a physical exam, a lung function test, and chest x-rays. ATSDR cannot pay for transportation costs to and from the evaluation. Special preparation or paperwork is not needed for these tests, but you will be asked to sign an informed consent form. Participation is voluntary.

Why?
A public health action by ATSDR in response to community concern about the health of former workers.

Who?
People who worked five or more years in processing phosphorus ore at the Tarpon Springs, Florida, plant. People who worked only in administrative or office positions are not eligible. A letter of invitation to participate in the testing will be mailed to eligible former workers in September, 2004. If you think you are eligible for this medical evaluation and did not receive a letter of invitation please call Steve Inserra of ATSDR toll-free at 1-888-422-8737.

Where & When?
Occupational Health Service of Helen Ellis Clinic, 2280 U.S. Highway 19, Holiday, Florida (telephone 727-942-2518) from October 5, 2004 through January 31, 2005.

How?
1. You receive ATSDR letter of invitation to participate in the medical evaluation.
2. You call the clinic to schedule an appointment for the ATSDR lung evaluation 727-942-2518.
3. You go to the clinic on your scheduled day and time and read and sign a medical consent form.
4. You perform simple, painless tests and procedures; allow up to 2 hours.
5. You meet with the doctor to discuss test results and receive a copy of your test results.

Protecting Your Privacy:
Test results will be kept confidential to the extent allowed by law. Your name will not be released to any person, insurance company, business, or media.

How Findings Will Be Shared With The Community:
ATSDR will share overall project findings  through a public meeting and a summary report.
