Standard Forms and Agreements

  • Cooperative Research and Development Agreement (CRADA) [PDF | Word] is used when NIH and Industry collaborate to further develop a technology for commercialization.

  • Letter of Intent (LOI) for CRADA [PDF | Word] - Prior to final approval of the CRADA, a Letter of Intent (LOI) may be signed allowing the research to proceed pending final negotiation and review.

  • Materials-Cooperative Research and Development Agreement (Materials CRADA) [PDF | Word] - is a simpler form of CRADA. It is limited in time to one year and does not allow for funds to be provided for support of research personnel, but does provide for the transfer of intellectual property rights to the collaborator.

  • Material Transfers - used for the exchange of materials with outside organizations for research purposes.   • Letter of Collection (LOC) - used for collecting materials from a foreign country for screening, evaluation and testing. Also see MOU - used when the foreign country provides extracts and compounds directly to the NCI.

  • Employee Invention Report (EIR) [PDF] | [Word]- used by NIH staff for reporting and evaluating inventions.

  • SAIC-Frederick Invention Report Coversheet [PDF] - For use by SAIC-Frederick staff only - complete and attach to EIR.

  • Confidential Disclosure Agreement (CDA) - used for the exchange of confidential information. There are 3 different versions of the CDA. Select from the following:
