Preparation of Electronic Submissions to FDA for Food and Color Additive Petitions:

Under the Draft Guidance For Industry: Providing Regulatory Submissions in Electronic Format Food and Color Additive Petitions, the Office of Food Additive Safety is providing the following electronic forms for petitioners to submit their petitions. The forms may be printed and mailed to the Office of Food Additive Safety.

Alternatively, the template versions of FDA Form 3503 and FDA 3504 may be downloaded to the user's computer for data entry. The completed forms may be submitted by electronic mail to: ELECSUB@cfsan.fda.gov.
