Fill-in Forms on the Copyright Office Website

The United States Copyright Office now offers fill-in versions of most copyright registration forms on the Copyright Office website.

An applicant for copyright registration or recordation may select the proper form online and key information directly onto the form instead of filling it in by hand or on a typewriter. After the form is filled in, it should be printed and mailed to the Copyright Office together with the deposit and the filing fee.

Forms should be printed head-to-head on a single piece of 8 1/2-inch by 11-inch white paper so that when you turn the paper over, the top of page 2 is directly behind the top of page 1. The Copyright Office requires all applications to arrive in the office on a single sheet of paper. Therefore, you should consider reinserting the first page of your competed application back into the printer so that the back page will be printed onto it.

Who May Use the Fill-In Forms?

Any applicant for copyright registration or recordation who has access to a computer with an Internet connection and a laser printer may fill in the form on the computer's screen.

Which Forms Are Available in Fill-In Version?

The forms currently existing in fill-in version include Forms TX, PA, SR, VA, SE, SE/Group, RE, RE/CON, Addendum to Form RE, GATT, GATT/GRP, GATT/CON, CA, CON, GR/PPh/CON, and Short Forms TX, PA, VA, and SE.

Where Are the Fill-In Forms?

The fill-in forms are on the Copyright Office website at www.copyright.gov. Under "Publications" click on "Forms."

May I Submit My Application Online?

At this time the Copyright Office does not accept online registration. You must print (see instructions above) your application after you have filled it in online and mail it to:

Can I Save the Filled-In Forms?

Copyright Office forms are distributed on our website in Adobe portable document format (pdf), a format that allows documents to be filled in, viewed, and printed with their original appearance preserved. Because pdf is a proprietary format created by Adobe Systems Inc., there are certain built-in restrictions on its use. The freely distributed Adobe Acrobat Reader allows for viewing and printing pdf files but not for saving forms. In order to save a completed form, you must use the full version of Adobe Acrobat, which can be purchased from Adobe or from many computer software vendors. We recognize that using a restricted format inconveniences some users, but we weigh that inconvenience against the ease-of-use of the fill-in features of pdf files.
