Instructions for Completing DTIC's Registration Form

1) See "Easy
Step" Instructions for a brief outline of registration procedures.
2) DoD organizations that choose either unclassified/unlimited or
unclassified/limited access, need to complete only Section I of the
DD Form 1540. In this case, you can choose to register by telephone,
email or fax! (Telephone: (703)767-8273; email: reghelp@dtic.mil;
fax: (703)767-9459) DoD organizations requesting classified access
must complete the entire form and may not register by telephone or
email. DD Form 1540 can be found at http://www.dtic.mil/dtic/registration/reg_forms.html.
3) Non-DoD Government organizations who request unclassified/unlimited
data need to complete only Section 1. In this case, you can choose

Section
I
(General
Information) |
Section I solicits specific
information about the requesting organization and the individual
designated to request and receive all shipments from DTIC.
Items #1-3. Complete in full.
Item #4. Attention. The individual who is registering with
DTIC® and is authorized to receive products and services. This
individual must be authorized to receive the level of information
indicated on the DD Form 1540 in Block 11, Type of Access Desired.
The security officer must provide documentation to support this
person's eligibility to receive all of the information requested.
Active military and DoD civilian employees must complete block 4.b.,
SSN. Although this information is voluntary, failure to provide
the information may result in the individual not being registered
for scientific and technical information services. This information
will be used to verify, through the Defense Manpower Data Center,
the individual's affiliation with a DoD organization. The Social
Security Number will not be retained in DTIC's files.
Items #5-6. Complete in full.
Item #7. Internet E-Mail Address. Include e-mail address
for individual identified in Item #4.
Item #8. Leave BLANK.
Item #9. Current or Former User. Are you a current or former
DTIC user? If you answer "yes," enter your present or
past DTIC User Code in the space provided.
Item #10. Leave BLANK.
Item #11. Type of Access Requested. Place an X in the appropriate
boxes.

Section
II
(Security Officer) |
Section II. All U.S. Government
or-ga-ni-za-tions must complete this section to obtain access to
classified information (Con-fi-den-tial, Secret) and/or to Restricted
Data, CNWDI or NATO information.
The Security Officer must provide documentation showing that the
individual on the attention line is eligible to receive all of the
information requested. He/she must provide a transmittal letter
with attachments. The subject of the transmittal should be "Transmittal
of Application for Registration w/Supporting Documentation".
The transmittal should state that the agency's supporting NATO Control
and Accountability is assigned to one of the following:
- Central Registry,
- Sub-Registry,
- Control Point
The letter should provide the person's
name and state that he/she is a U.S. Citizen who has a need-to-know
and has been briefed and granted access to classified information
up thru Confidential/Secret; to include NATO, CNWDI, and Restricted
Data, if appropriate. The letter should include the level of the
requestor's classified storage capability and classified mailing
address. The letter must be signed by one of the following officials:
- POC's Commander
- Security Officer
- Approving Official
Item #12. Security Officer.
Complete all items. Obtain signature of your organization's security
officer. The security officer certifies that the organization
may receive and store classified data at the access level indicated
in Section I. (See required information above.)

III
(Prime Contractor
Approval)
|
Section III-leave

Prime contractor approval is no longer required.

Section
IV
(Government
Approving Official) |
Section IV must
be completed by DoD organizations requesting access to classified
information and Non-DoD Government organizations requesting access
to limited or classified information.
Items #17-18. Complete in full.
Approving Official for DoD organizations is the Commanding
Officer, Technical Director or other authorized designee. In the example
below, DTIC's Directors are the authorized designees to sign the DD
Form 1540. DTIC is shown here only as an example to show how a DoD
organization would complete Section IV.
Non-DoD Government organizations should refer to Appendix A,
"Non-DoD Offices Designated to Approve DD Form 1540" to
find the appropriate Government approving official.
Item #19. To be completed by the Government approving official.
His/her signature is required.

Section
V
(Subject Fields and Groups)- |
Section V must be
completed if your organization is requesting
access to classified information (Confidential, Secret) and/or
to Restricted Data, CNWDI or NATO information.
If you are requesting access to an entire subject field, place an
X in the first block of that field (indicating ALL). However, if less
than the entire subject field is required, place an X in the block
for each subject group required.

Forwarding
the DD Form 1540 |
DoD organizations requesting access
to unclassified/unlimited or unclassified/limited data, should
forward the form directly to DTIC after completing Section I. It
must be mailed or faxed to the address and fax numbers as follows:

Receipt
of DTIC
User Code |
Once your registration
is processed by DTIC, you will receive a welcoming letter via email.
This letter will include your DTIC user code. You will also receive
a package via regular mail that will include a copy of your processed
DD Form 1540 and other information about DTIC.
