NOTICE ON CITY OF CHICAGO AND SUBURBAN COOK COUNTY UNIFORMED SERVICES VOTING PROJECT

THE CITY OF CHICAGO AND SUBURBAN COOK COUNTY ARE PROVIDING ELECTRONIC DELIVERY OF BLANK BALLOTS FOR THE NOVEMBER 2, 2004 GENERAL ELECTION

The City of Chicago/Suburban Cook County will deliver blank ballots to Uniformed Service voters and accompanying family members by FAX or E-MAIL for the November 2, 2004 General Election. These jurisdictions will accept requests for absentee ballots (Federal Post Card Application) by fax or e-mail and will send out absentee ballots by fax and e-mail. Completed ballots must be returned by regular mail no later than 7 p.m. (Central Time) on Election Day. If you are a military service member or an accompanying family member and wish to participate in this program, follow the directions below.

OBTAINING A FEDERAL POST CARD APPLICATION: You can obtain your Federal Post Card Application (FPCA) from your Voting Assistance Officer or you can download the on-line form from the FVAP website at www.fvap.gov and fill it out on-line. You must then print the completed form, sign, date it and return it by fax, as described below. After faxing, you must mail the FPCA to your Local Election Official.

CITY OF CHICAGO VOTERS:

FAX DELIVERY: Submit the FPCA by FAX to the FVAP Electronic Transmission Service (ETS) at DSN 223-5527 or commercial 703-693-5527 or toll free within the U.S. 800-368-8683. The ETS can also be accessed toll-free from 51 countries. Refer to the FVAP website at http://www.fvap.gov/services/faxing.html for these fax numbers. Be sure to provide your commercial or DSN FAX number at Item 5 of the FPCA. The FVAP ETS will forward your FAX directly to the Election Board. Although we prefer that you use the FVAP ETS, you may also FAX the FPCA directly to the Board at 312-269-1624. The FPCA must be mailed after faxing to the City of Chicago, Board of Election Commissioners, 69 West Washington Street, Suite 600-800, Chicago, Illinois 60602-3012.

The FPCA must be received no later than 10 days before the election. Your ballot materials will be sent to the FAX number you provide. The completed ballot and certification MUST BE MAILED BACK and must be received no later than 7 p.m. (Central Time) on Election Day.

E-MAIL DELIVERY: Send an e-mail to the Chicago Board of Elections at absentee@ameritech.net. You must include your legal Chicago voting address, your current mailing address and your e-mail address. Your ballot materials will be sent to that e-mail address. The completed ballot and certification MUST BE PRINTED AND MAILED BACK and must be received by the Board no later than 7 p.m. (Central Time) on Election Day.

SUBURBAN COOK COUNTY VOTERS:

Although we prefer that you use the FVAP ETS, you may also FAX the FPCA directly to the Clerk's Office at 312-603-9784. The FPCA must be mailed after faxing to the County Clerk, Cook County Board of Election Commissioners, 69 West Washington Street, Suite 500, Chicago, Illinois 60602-1304.

Your ballot materials will be sent to that e-mail address. The completed ballot and certification MUST BE PRINTED AND MAILED BACK and must be received by the Board no later than 7 p.m. (Central Time) on Election Day.

In the U.S. call toll-free at 1-800-438-VOTE (8683), local (703) 588-1584 or use DSN 425-1343. The VIC provides recorded messages from candidates, as well as other information concerning elections, 24 hours a day.
