Malaria Case Surveillance Report Form

CDC provides a Malaria Case Surveillance Report FormAdobe Acrobat Reader (119 KB/2 pages) with DirectionsAdobe Acrobat Reader(66 KB/2 pages). Print and mail the completed form to the address at the top of the surveillance form or fax it to CDC, Malaria Branch, at 770-488-4206.

Malaria is a reportable disease in the United States and its territories. Health-care providers should report all cases of laboratory-confirmed malaria occurring in the United States and its territories to their local or state health department. Reports are transmitted to CDC through the National Malaria Surveillance System (NMSS). Surveillance forms may be sent to CDC's Malaria Branch either by mail (at the address at the top of the form) or by fax (at 770-488-4206).
