Resources for Pre-Deployment

You just received orders that you are to deploy; many arrangements need to be made both in your personal and professional life. The goal of this web page is to make this process a little easier by providing pre-deployment resources to address areas of concern.

    Medical Screening (DD-2795)

    The Pre-Deployment Health Assessment form is a self-report form. On this form, you are asked several questions about your medical and dental conditions. The goal of this form is to provide baseline medical information needed to determine your deployable status. When completing this form, it is very important that you take time to reflect on your medical status. You will be asked when you last had a physical exam, dental exam and eye exam. This assessment is designed as a safeguard for your health before you depart for deployment. So be sure to complete it and submit it to the medical staff designated to assist with your deployment process.

    Referrals will be made to appropriate providers for further assessment when indicated. A health professional will review this form with you and answer any questions. One copy of this form will accompany you to your deployment in your deployable medical folder, the DD Form 2766- Adult Preventative and Chronic Care Flowsheet. One copy of this form should be filed in your permanent medical record and one copy should be mailed by medical personnel to AMSA at:

    Immunizations

    Required immunizations differ by geographic region. The specific immunizations will be determined when you do your pre-deployment in processing. You will be given a deployable medical folder (DD Form 2766- Adult Preventative and Chronic Care Flowsheet) that is a condensed health record containing your essential health information. This record includes your immunization information as well as your latest HIV and DNA collection date. You should take this folder with you to your deployment site. You should also have a copy of your DD2795-Pre-Deployment Health Assessment in this folder to take with you when you deploy, where it should remain until you redeploy back to your home station.
