THIS NOTICE CONSTITUTES AMENDMENT NO. 4 TO THE COMBINED SYNOPSIS/RFQ FOR LINEARLY POLARIZED SINGLE FREQUENCY DIODE LASER. Companies shall acknowledge all amendment(s) in their quote. This notice serves as the official amendment to subject synopsis/RFQ and a written amendment will not be issued.

The purpose of this amendment is to extend the quote due date to October 1, 2004 and correct a duplicate numbering sequence of previously posted amendments, correction is as follows:

Duplicate numbered Amendment 1 dated August 30, 2004 is hereby corrected to read: Amendment 2. Amendment 2 dated September 9, 2004 is hereby corrected to read: Amendment 3.

Thus, there have been 3 Amendments posted to this solicitation prior to the date of this Amendment 4.

Offerors are reminded to acknowledge in their quotes receipt of all amendments.

The due date for receipt of offers is extended to October 1, 2004.

Companies shall provide the information stated in the synopsis/RFQ posted on the NASA Acquisition Internet Service (NAIS) on August 12, 2004. Documents related to this procurement are available over the Internet. These documents reside on a World Wide Web (WWW) server which may be accessed using a WWW browser application.
