The 2003 model CFC pledge card is attached and will be posted on the CFC website for downloading and reproduction. Please note that there are three important changes to the document this year.
First, the Payroll Deduction Authorization statement has been revised to list calendar year "2004". The Office of CFC Operations received suggestions from campaigns to delete references to the year so that pledge cards could be used in more than one campaign year. We explored that option but found such a change may result in significant difficulties for payroll offices. Of particular concern, omission or "write-ins" of the year may lead to difficulties in the tracking and verification of CFC payroll deductions to a calendar year. We will continue to assess alternatives and welcome suggestions from LFCCs and PCFOs.
Second, the language in the "Unit/Division or Payroll Office No." box has been revised to state "Unit/Division and Payroll Office No." Every payroll office has a unique eight digit identification number. This payroll office number is assigned by OPM and is usually the same as the agency's Agency Locator Code (ALC) assigned by Treasury. For example, OPM's payroll office number is 24000001, GSA's National Payroll Center's payroll office number is 47000016, and the U.S. Postal Service's payroll office number is 18000009. As such, staff in the Office of CFC Operations, which is part of the Office of the Director (OD), would enter "OD (the unit/division) 24000001 (payroll office no.)" in the box.
OPM strongly encourages campaigns to require keyworkers or agency coordinators to include this number on every pledge card. Their payroll office will be able to provide the information. We also encourage PCFOs to record this information in their database system. This can be a valuable tool in tracking receipts from each of the Federal agencies in your campaign area. In cases where the agency is part of a payroll center covering multiple agencies, the number can be referenced to identify the amounts being transmitted from each agency. Finally, LFCCs are being given the discretion to determine if the Social Security Number (SSN) box on the PCFO and donor copies will no longer be blocked out. In previous years, OPM did not permit campaigns to record the SSN in any area other than on the copy that was submitted to the payroll office. At the request of many campaigns and in accordance with the Privacy Act statement on the back of the pledge card, OPM concluded that this information may be collected by the PCFO for use only as a tool to identify individual donors, when corrections or clarifications regarding the pledge card information are needed. In compliance with the Privacy Act, the inclusion of a donor's SSN cannot be made mandatory.
Campaigns must use this model pledge form as the basis for designing pledge forms locally. While campaigns may incorporate additional giving levels and include their award recognition programs, no further modifications are permitted unless approved in advance by OPM.
