Documents required for application

  1. Original visa fee receipt issued by Banco Union Colombiano. In Bogota, the visa application fee must be paid at the Banco Union Colombiano located at Carrera 47 # 22A-99 (Edificio Eliana) or Sucursal Avenida Jimenez (Carrera 8 # 14-45) or Sucursal Avenida Chile (Carrera 7 # 71-48). In Cali, Medellin, Barranquilla, Cartagena, Bucaramanga, Pereira and Manizales payment may be made at the main office of the Banco Union Colombiano. In all cases, it is necessary to present the applicant's passport at the Bank and the additional form for visa application. The receipt is non-transferable and valid until the date of the interview. The Embassy cannot reimburse for loss, robbery, double payment, or expiration of the receipt. The fee (payable in cash) is the peso equivalent of $100.00 dollars (depending upon the exchange rate), for each application. The receipt is adhesive and contains a barcode which allows it to be attached to the passport. Do not remove the receipt from your passport unless you are traveling outside of Colombia. In such a situation we recommend you keep the receipt in a safe place. Do not travel with the barcode receipt on your passport; immigration agents may not know what it is and may remove it.

  2. Current valid passport and previous passports (if any). Passport must be valid for at least six months after entry into the U.S.

  3. Colombian I.D. card. (cedula de ciudadania, tarjeta de identidad, or cedula de extranjeria).

  4. One visa application form (DS-156 08-2001) provided by the Banco Union Colombiano when the fee is paid. It must be filled out completely on both sides of the sheet, printed with the correct information, signed and dated by the applicant.

  5. One recent, color, 5x5cm. photograph, not blue background. Black and white photographs are not accepted.

  6. If you have children, authenticated copy of the folio of the Registro Civil de Nacimiento. Only the parents may apply on behalf of a minor child.

  7. All male non-immigrant visa applicants between the ages of 16 and 45, regardless of nationality and regardless of where they apply, must submit to the post a completed Form DS-157 in addition to the usual non-immigrant visa application (DS-156). Form DS-157 may be obtained in Bogota at the Banco Union Colombiano located at Carrera 47 # 22A-99 (Edificio Eliana) or Sucursal Avenida Jimenez (Carrera 8 # 14-45) or Sucursal Avenida Chile (Carrera 7 # 71-48). In Cali, Medellin, Barranquilla, Cartagena, Bucaramanga, Pereira and Manizales forms may be obtained at the main office of the Banco Union Colombiano.


Applicants for tourist visas who are 13 years of age or younger do not require a personal interview, unless the consular officer determines otherwise.
