Are there model enrollment forms for the private drug companies providing services for the Medicare drug discount card?

Yes. Go to the web site at www.cms.hhs.gov/discountdrugs and you’ll see the link in the box that says “highlights”. The companies can use this form or customize it. They must accept the standardized form if they receive it.
