  How will the geographic areas be selected under Phase I of the Chronic Care Improvement Programs?
  Answer
  Applicants will propose the geographic region(s) they wish to serve. CMS will provide applicants with a de-identified nationally representative sample dataset of the type of beneficiaries who would be included in this pilot. CMS reserves the right to ask applicants to adjust their proposed area to ensure that an appropriate population is served. More detailed information about this process will be available on the CMS website and will be discussed at length at the bidders conference.
