  What process is CMS using to provide enrollment forms to beneficiaries? Are these forms available online or does the beneficiary have to contact the approved card sponsors?
  Answer
  Standard enrollment forms can be obtained through the local SHIP representatives or by calling 1-800-MEDICARE. For those with access, www.medicare.gov has all the information needed to decide on the most appropriate drug card. Enrollment forms are available at www.medicare.gov. Enrollment forms customized for particular card programs can be obtained directly from the card programs.
