  What is the effective date of the determined prorated amount for the $600 credit for 2005? Is it the date of the application, the date it was received by the sponsor, or the date that the application is approved by CMS for the prorated amount?
  Answer
  The effective date for pro-ration is calculated from the date that the completed application is received by the sponsor. If the completed application is received in one quarter, but approved in another, the pro-ration is still based on the date the completed application was received. Enrollment in the card becomes effective the month after the application is received.
