Download the most frequently requested forms for manual
filing or for reference. Note that some downloadable forms in the PDF
format are “fillable” with the Acrobat reader; that is, they allow you
to fill out the form on-screen for printing and saving. To enable this
feature, you must have version 6 of the free Acrobat
Reader.
ULS is a purpose driven electronic filing system. Once
your application purpose and radio service codes are chosen, the correct
online forms will appear on the screen.
Online filing is strongly encouraged. The FCC's Universal
Licensing System (ULS) provides Wireless Radio Services applicants and
licensees an easy and efficient method to complete necessarily filings
with the FCC without having to complete paper forms.
