Program name Am I eligible?
Spina Bifida Health Care Program Am I eligible?

Eligibility criteria
In order to qualify for this benefit program, you must be the spouse or surviving dependent of a veteran (alive or deceased), you or your parent must have provided military service in Vietnam, and you or any of your family/household members must be facing Spina Bifida (except Spina Bifida Occulta).

Description
VA provides a health care program for natural children diagnosed with spina bifida (except spina bifida occulta) born to certain Vietnam veterans. Coverage under this program is based on eligibility determinations made by the VA Regional Office. Claimants submit applications on VA Form 21-0304. If the VA Regional Office grants a monthly monetary award for an individual with spina bifida, no separate application for health care benefits from the beneficiary is needed. The VA Regional Office forwards the award information to the VA Health Administration Center, which contacts and registers the beneficiary for health care benefits for treatment of spina bifida and related medical conditions.
