Eligibility criteria
In order to qualify for this benefit program, you must be uninsured (no health insurance) or underinsured. You must also characterize your income as low or very low and you must meet one of the following criteria: 1) be homeless/live in a shelter, 2) live in public housing, 3) be a migrant or seasonal farmworker, or 4) your household's annual income before taxes must be less than $17,721 if one person lives in the household; less than $23,881 if two people live in the household; less than $30,041 if three people live in your household; less than $36,201 if four people live in your household; less than $42,361 if five people live in your household; less than $48,521 if six people live in your household, less than $54,681 if seven people live in your household; less than $60,841 if eight people live in your household; less than $67,001 if nine people live in your household; less than $73,161 if 10 people live in your household; less than $79,321 if 11 people live in your household; less than $85,481 if 12 people live in your household; less than $91,641 if 13 people live in your household; less than $97,801 if 14 people live in your household; and less than $103,961 if more than 14 people live in your household.
Description
Community Health Centers offer access to comprehensive primary and preventative health care and focus primarily on low-income and underserved individuals. Services are offered via community health centers, migrant health centers, health care for the homeless programs, public housing primary care programs, and school-based clinics. Eligibility for services, as well as fees charged for services, are determined when a patient visits a health center and are generally based on income and ability to pay.
